2011 U.S. Synchronized Skating Championships

Volunteer Information and Application

Thank you for your interest in volunteering at the 2011 U.S. Synchronized Figure Skating Championships
in Ontario, California. This letter will outline some basic information regarding the volunteer
requirements and will be followed by the application. You may either complete the application on line or
submit by printing and mailing.

• You must have a form on file to be accepted as a volunteer.
• You must be at least 18 years of age.
• You will need to supply an e-mail address for contact purposes.
• Volunteers must commit to a minimum of 2 six-hour shifts.
• Dates, Times and Areas to work are requests only; there is no guarantee of requests for
  spots.

You will receive confirmation of registration as a volunteer via your e-mail address.
You will be contacted regarding scheduling of shifts at a later date.

(Initial) In order to participate as a volunteer for the 2011 U.S. Synchronized Figure Skating
Championships, I understand I will be required at a later date to sign both an authorization to a full
criminal background check prior to my assignment and a volunteer waiver release and hold harmless
agreement prior to my assignment with the 2011 U.S. Synchronized Figure Skating Championships.
